Mental Health Coaching Request Form

Please fill out this form if you would like to receive assistance from a Mental Health coach at Generation Church. We look forward to serving you!

You must be 18 or older to receive coaching. We recommend The Landing, part of our Celebrate Recovery Ministry, which meets at 6 pm on Tuesday nights at the Mesa campus. This is a great ministry for 6th-12th graders looking for healing from any hurt, habit, or hang-up.

We cannot accept requests on behalf of others. This form must be completed by the person requesting to connect with a Mental Health coach.

We are not licensed therapists. We are here to help give you biblical tools and skills to help you through a difficult season. However, if you are seeking a licensed Christian therapist, we recommend Professional Counseling Associates of Arizona. You can make an appointment at pcaaz.com.
